Request a Transcript

Transcripts of your classes will be provided upon written request submitted to the Dean's Office. Please note that we will be unable to honor your request if there is an outstanding balance on your student account at the end of each school year.

A printable version of the Transcript Request Form is available. This form must have a parent signature. An electronic version is not acceptable. Please mail your request to the following address:
